Abstract
The aim of this study was to compare balance performance in two groups of women with upper cervical spine dysfunction following whiplash-associated disorders (WAD) of grades I–II. One group consisted of women (n=12) with recent symptoms of WAD (10–12 weeks post collision) and the other group consisted of women (n=20) with chronic symptoms of WAD (>1 year). Asymptomatic women (n=30) served to give baseline data. Balance performance was measured with dynamic posturography (SMART Balance Master from Neuro- Com International Inc.). The results revealed balance disturbances in the WAD groups compared with the control group (p<0.001). Moreover, the women with chronic WAD showed greater balance disturbances than the women with recent WAD (p=0.04). The results of this study indicate that it is important to evaluate women with WAD and upper cervical spine dysfunction early on with respect to balance disturbances. Cervical-induced balance disturbances may be an important key to understand why some individuals with WAD enter the chronic stage. An appropriate balance-retraining program may prevent development and maintenance of chronic symptoms.
Markmið rannsóknarinnar var að bera saman jafnvægisviðbrögð hjá tveimur hópum kvenna með hreyfitruflanir frá efri hálshrygg í kjölfar hálshnykks af gráðu I–II. Í öðrum hópnum voru konur (n=12) með nýleg einkenni (10–12 vikur frá áverka) og í hinum hópnum konur (n=20) með langvarandi einkenni (>1 ár). Einkennalausar konur (n=30) gáfu grunnviðmið. Jafnvægisviðbrögð voru mæld með „dynamic posturography“ (SMART Balance Master frá NeuroCom International Inc.). Niðurstöðurnar leiddu í ljós að hálshnykks- hóparnir höfðu verra jafnvægi en viðmiðunarhópurinn (p<0.001). Þar að auki sýndu konurnar sem voru með langvarandi einkenni meiri jafnvægistruflanir en konurnar með nýleg einkenni (p= 0.04). Niðurstöðurnar benda til að það sé mikilvægt að meta konur með hálshnykksáverka og hreyfitruflanir frá efri hálshrygg snemma með tilliti til jafnvægis. Jafnvægistruflanir sem stafa frá efri hálshrygg geta verið mikilvægur þáttur til að skilja hvers vegna sumir þeirra sem hafa fengið hálshnykk fá langvarandi einkenni. Viðeigandi jafnvægismeðferð gæti komið í veg fyrir að einkenni og afleiðingar verði eins langvarandi og erfið og raun ber vitni.
