Abstract
Lung transplantation is an option in the treatment of end stage lung diseases, excluding lung cancer, that lead to short life expectancy and poor quality of life. Now they are mostly limited by shortage of donor organs and longterm complications. They are used for various lung diseases such as pulmonary vascular diseases, fibrosing diseases, chronic obstructive pulmonary diseases and diseases that cause chronic infections. Depending on the indication it is possible to perform heart and lung transplantation, single lung or double lung transplantation.Indications, contraindications, surgical methods, immunosuppression, complications and outcomes will be discussed. Survival is not as good as for other solid organ transplantation. Measurement of pulmonary function and quality of life improve with lung transplantation. Bronchiolitis obliterans is the most common complication and is the most limiting factor. A few Icelanders have undergone lung transplantation, most of them in Gothenburg, Sweden. The future of lung transplantation depends on limiting the incidence of bronchiolitis obliterans and finding more organ donors.
Lungnaígræðsla er valkostur við meðferð langt gengnum lungnasjúkdómum, öðrum en krabbameinum, þar sem lífslíkur eru mjög skertar og lífsgæði léleg, þrátt fyrir bestu lyfjameðferð. Þær takmarkast mest af skorti á líffæragjöfum og langtíma fylgikvillum. Þeim er beitt við ýmsum gerðum af lungnasjúkdómum, svo sem lungnaæðasjúkdómum, bandvefsmyndandi sjúkdómum, langvinnum lungnateppusjúkdómum og sjúkdómum sem valda þrálátum sýkingum eins og slímseigjusjúkdómi (cystic fibrosis). Hægt er að græða í bæði hjarta og lungu, annað lunga eða bæði lungu eftir sjúkdómsgerð. Sagt er frá ábendingum, frábendingum, skurðtækni, ónæmisbælandi lyfjum, fylgikvillum og árangri. Lifun er ekki eins góð og í öðrum líffæraígræðslum og stafar það af því hve lungun eru viðkvæm. Lungnapróf og heilsutengd lífsgæði batna þó mikið við lungnaígræðslu og sjúklingar losna við súrefnisgjöf. Stíflumyndandi berkjungabólga er algengasti fylgikvillinn og takmarkar langmest árangur af lungnaígræðslum. Nokkrir Íslendingar hafa gengist undir lungnaígræðslur, flestir í Gautaborg. Framtíð lungnaígræðslu snýst um að draga úr tíðni stíflumyndandi berkjungabólgu og að afla fleiri líffæragjafa.

Export search results
