Abstract
Introduction: Varicella is a common disease with possible consequences. The disease is usually contracted in childhood and persistent antibodies are produced. Varicella vaccine is not widely used in Iceland. The aim of the study was to investigate the epidemiology of varicella in Icelandic children and it s complications. The results may prove important when deciding on varicella immunization in Iceland. Material and methods: The study was retrospective cross sectional. Varicella antibodies were measured from children <18 years of age. Hospital records of children with varicella were evaluated. Results: Serum samples numbered 280. Varicella antibodies were present in 65% of children <1 year of age but in 10% of 1-2 year old children. Almost all children had antibodies before the age of ten and 97.5% of children >10 years had antibodies. Hospital admittions were 58, annual admittions were or 3.6/100.000 children <18 years. Bacterial infections, most frequently skin infections, were the most common complication, followed by cerebellar ataxia, dehydration and nutritional deficiency. Conclusion: Most children in Iceland contract varicella before the age of ten. Substantial complications were recorded. It is important to recognize varicella, it's complications, treatment options and explore whether varicella vaccination should be instigated in Iceland.
Tilgangur: Hlaupabóla er algengur sjúkdómur og getur haft alvarlegar afleiðingar. Börn fá helst sjúkdóminn og mynda langvarandi mótefni. Bóluefni gegn hlaupabólu hefur lítið verið notað á Íslandi. Markmið rannsóknarinnar var að meta faraldsfræði hlaupabólu hjá íslenskum börnum og fylgikvilla hennar. Niðurstöður gætu meðal annars gagnast við ákvörðunartöku um bólusetningar gegn hlaupabólu á Íslandi. Efniviður og aðferðir: Rannsóknin var aftur-virk, þversniðsrannsókn. Mæld voru mótefni gegn hlaupabólu í blóðvatnssýnum frá börnum <18 ára sem safnað hafði verið á árunum 2001-2002. Fylgikvillar voru kannaðir í sjúkraskrám barna sem lögð höfðu verið inn á barnadeildir í Reykjavík á 20 ára tímabili. Niðurstöður: Fjöldi sýna var 280. Á fyrsta aldursári voru um 65% barna með mótefni gegn hlaupabólu en um 10% barna á aldrinum 1-2 ára. Fyrir 10 ára aldur voru nær öll börnin með mótefni, samanlagt voru 97,5% (78/80) barna >10 ára með mótefni. Börn sem lögð voru inn vegna hlaupabólu eða fylgikvilla voru 58 eða 3,6 /100.000 börn á ári. Bakteríusýkingar voru algengasta ástæða innlagnar, einkum húðsýkingar en hnykilslingur, þurrkur og vannæring voru einnig algeng. Ályktun: Flest börn á Íslandi fá hlaupabólu fyrir 10 ára aldur. Fylgikvillar geta verið alvarlegir. Mikilvægt er að þekkja sjúkdóminn, viðbrögð við honum og kanna hvort hefja eigi almenna bólusetningu gegn honum hér á landi.
