Abstract
Á Íslandi hafa hjúkrunarfræðingar ekki leyfi til að gefa lyf án ávísunar frá lækni en lyfjagjöf hjúkrunarfræðinga án milligöngu læknis þekkist þó í bráðaþjónustu hérlendis. Tilgangur rannsóknarinnar var að varpa ljósi á umfang og eðli stakra lyfjagjafa hjúkrunarfræðinga án skriflegra fyrirmæla lækna á Landspítala. Um lýsandi afturvirka megindlega rannsókn var að ræða. Í þýðinu voru allar ávísaðar og skráðar lyfjagjafir á Landspítala í rafræna lyfjaumsýslukerfinu Therapy sem voru 1.586.684 árið 2010 og 1.633.643 árið 2011. Í úrtakinu voru stakar lyfjagjafir hjúkrunarfræðinga á Landspítala án skriflegra fyrirmæla lækna, sem voru skráðar í rafræna lyfjaumsýslukerfið Therapy undir heitinu umbeðið af hjúkrun árin 2010 og 2011, samtals 24 mánaða tímabil. Gögn voru fengin frá heilbrigðis- og upplýsingatæknisviði Landspítala að fengnum viðeigandi leyfum. Við úrvinnslu voru gögnin lesin inn í Microsoft Excel með Power Pivot viðbót. Árið 2010 voru stakar lyfjagjafir 4% (n=63.454) af heildarlyfjagjafafjölda á Landspítala og 4,23% (n=69.132) árið 2011. Fjöldi stakra lyfjagjafa jókst um 8,95% frá 2010 til 2011 og er það marktækur munur á árum χ2 (1, N=159.586) = 243,46, p<0,001. Flestar stakar lyfjagjafir hjúkrunarfræðinga voru á skurðlækningaog lyflækningasviðum bæði árin. Mest var um stakar lyfjagjafir hjúkrunarfræðinga um kl. 22 á kvöldin. Flestar stakar lyfjagjafir hjúkrunarfræðinga komu úr lyfjaflokki N (m.a. verkjalyf, svefnlyf og róandi lyf), 65,34% árið 2010 og 65,03% árið 2011, lyfjaflokki A (m.a. ógleðistillandi lyf og sýrubindandi lyf), 15,70% árið 2010 og 16,78% árið 2011, og lyfjaflokki M (m.a. bólgueyðandi gigtarlyf), 6,23% árið 2010 og 5,38% árið 2011. Þessar niðurstöður benda til þess að stakar lyfjagjafir hjúkrunarfræðinga án skriflegra fyrirmæla lækna séu nokkuð algengar á Landspítala. Niðurstöðurnar krefjast þess að viðfangsefnið sé skoðað nánar og verklag við lyfjavinnu á bráðasjúkrahúsum endurskoðað.
In Iceland nurses are not licensed to prescribe medications, but in practice it is known that nurses administer medications in acute care without doctors’ prescriptions. The purpose of this study was to identify the extent and characteristics of medication administration by nurses without doctors’ prescriptions at Landspitali University Hospital. This was a descriptive, retrospective and quantitative study. The study population were all prescriptions that had been registered into the electronic medication management system, Therapy, at Landspitali, in total 1,586,684 in 2010 and 1,633,643 in 2011. The sample comprised individual medications administered by nurses without doctors’ written prescriptions that had been registered in Therapy as prescribed by nursing in the years 2010 and 2011, or 24 months in total. Data were provided by the Department of Health and Information Technology at Landspitali after appropriate permissions had been granted. The data were transferred into Microsoft Excel with Power Pivot addition for analysis. In the year 2010 individual medication administrations by nurses were 4% (n=63,454) of the total number of medication administrations at Landspitali and 4.23% (n=69,132) in the year 2011. The number of individual medication administrations by nurses increased by 8.95% between 2010 and 2011, indicating a statistically significant increase (χ2 (1, N=159,586) = 243.46, p<0.001). Most individual medication administrations by nurses were in the surgical and medical divisions in both years. Individual medication administrations by nurses occurred most often around 10 pm. Most individual medication administrations by nurses were from the ATC class N (such as analgesics, hypnotics and sedatives) accounting for 65.34% in 2010 and 65.03% in 2011; from ATC class A (such as antiemetica and antacids) which accounted for 15.70% in 2010 and 16.78% in 2011; and from the ATC class M (such as anti-inflammatory and anti-rheumatic products) accounting for 6.23% in 2010 and 5.38% in 2011. These results indicate that individual medication administrations by nurses without doctors written prescriptions are significant in Landspitali. The results require further exploration of medication work and work procedures in acute care.
