Loftbrjóst beggja vegna eftir nálastungumeðferð á meðgöngu - sjúkratilfelli

Abstract

Hér er lýst tilfelli konu á fertugsaldri sem leitaði á bráðamóttöku vegna vaxandi takverks og mæði nokkrum klukkustundum eftir nálastungumeðferð. Nálastungurnar fékk hún vegna meðgönguógleði og uppkasta en hún var þá komin tæpar 15 vikur á leið. Við komu var hún með hvíldarmæði, aukna öndunartíðni og hjartsláttarhraða en súrefnismettun og blóðþrýstingur innan viðmiðunarmarka. Við hlustun voru skert öndunarhljóð yfir lungnatoppum og lungnamynd sýndi nánast algjört samfall á báðum lungum. Komið var fyrir brjóstholskerum beggja vegna sem fjarlægðir voru einum og tveimur dögum síðar og hún útskrifuð heim. Konunni heilsaðist vel eftir útskrift og meðgangan gekk vel í kjölfarið. Þetta tilfelli sýnir að loftbrjóst getur hlotist af nálastungumeðferð ef nálunum er stungið of djúpt í brjóstkassann. Í þessu tilviki hlaust af loftbrjóst beggja vegna sem getur reynst lífshættulegt.

A woman in her thirties, 15 weeks pregnant, underwent acupuncture therapy because of pregnancy-related nausea and vomiting. Several hours later she experienced shortness of breath and therefore came to the emergency room. Physical examination revealed tachypnea and reduced breath sounds bilaterally but normal oxygen saturation and blood pressure. Chest X-ray showed bilateral subtotal pneumo­thoraces. Chest tubes were inserted into both pleural cavities and the patient recovered successfully and was discharged in good health 3 days after admission. This case report emphasizes the risks of acupuncture to the chest cavity that in this case resulted in bilateral pneumothoraces, a condition that can become life threatening in this case to both mother and fetus.
