Abstract
Mat á stýrifærni (e. executive function) er mikilvæg viðbót við greiningu á ADHD hjá börnum og unglingum. Stýrifærni vísar meðal annars til þátta eins og vinnsluminnis, sjálfsstjórnar, tilfinningastjórnar og skipulagsfærni. Tilgangur rannsóknarinnar var að meta próffræðilega eiginleika íslenskrar þýðingar á BRIEF (e. Behavior Rating Inventory of Executive Function) meðal foreldra íslenskra grunnskólabarna. Tvær samliggjandi rannsóknir voru framkvæmdar; annars vegar Rannsókn I þar sem athugaðir voru próffræðilegir eiginleikar BRIEF-listans og framkvæmd staðfestandi þáttagreining þar sem foreldrar 600 barna í 4.–10. bekk í sjö grunnskólum á höfuðborgarsvæðinu svöruðu listanum. Hins vegar var framkvæmd Rannsókn II þar sem BRIEF-listinn var notaður í hópi 8–11 ára barna með ADHD til að skoða breytingar fyrir og eftir Snillinganámskeið sem þau tóku þátt í. Niðurstöður sýndu að áreiðanleiki og endurprófunaráreiðanleiki íslenskrar þýðingar BRIEF-listans reyndist góður og samleitni réttmæti við sambærilega lista var gott. Framkvæmd var staðfestandi þáttagreining þar sem notast var við ML-matsaðferð (e. maximum likelihood estimation) og niðurstöður bornar saman við upprunalega bandaríska útgáfu listans. Niðurstöður sýndu að gögnin féllu þokkalega að ætlaðri þáttauppbyggingu BRIEF-listans. Viðbótargreining var framkvæmd til að varpa frekara ljósi á uppbyggingu listans. Gagnsemi BRIEF-listans var metin til að skoða breytingar á stýrifærni í lok Snillinganámskeiðs. Einnig voru skoðaðar breytingar á ADHD-einkennum og kom fram að foreldrar mátu athyglisbrestseinkenni minni eftir námskeiðið. BRIEF-listinn getur verið gagnlegt mælitæki til að veita skilvirkari ráðgjöf og til að meta árangur meðferðar fyrir börn og unglinga með frávik í hegðun, þroska og líðan.
Assessing children´s executive function skills is an important addition to any ADHD diagnostic protocol. Executive functions refer to a set of cognitive skills such as working memory, self-control, emotion regulation, organizational skills, and more. Two studies were conducted to examine the Icelandic version of the BRIEF questionnaire (Behavior Rating Inventory of Executive Function). The purpose of Study I was to assess psychometric properties of the BRIEF among parents of 600 children from 4th–10th grade in seven elementary schools in the Reykjavik municipal area and to conduct a confirmatory factor analysis. The purpose of Study II was to use the BRIEF to assess changes in executive function skills following the Out- SMARTers program, a treatment program designed for children with ADHD. Results indicated that reliability and test-retest reliability of the Icelandic version of the BRIEF was very good, as well as convergent validity with other comparable measures. Confirmatory factor analysis based on the maximum likelihood estimation method revealed that the data fit the original eight factor model reasonably. Supplemental analysis was conducted to shed additional light on the factor structure. Results from the OutSMARTers program also demonstrated the usefulness of the BRIEF in terms of assessing changes in executive functions following treatment as well as parents rating overall improvement in attention skills. The BRIEF could therefore be a useful measure to plan treatment more effectively and assess treatment progress for children and teenagers with developmental, behavioral and emotional difficulties.
