Abstract
To explore the relationship between maternal s-AFP and fetal trisomy 21 in the second trimester in Iceland and to establish normal values for s-AFP for the National Hospital Biochemical Laboratory, s-AFP values for 1309 women that had undergone second trimester amniocentesis were examined retrospectively and the results presented as »multiples of medians« for different weeks of gestation. These values and the outcomes of the amniocentesis with regard to trisomy 21 were compared to risk tables presented by Cuckles et al, combining age and s-AFP risk. Down syndrome was found in ten out of 763 pregnancies (1.3%) with increased combined risk, compared with one Down syndrome in 546 pregnancies (0.2%) with decreased combined risk. It is suggested that screening for fetal chromosomal anomalies in all age groups should be considered in Iceland, either by s-AFP alone or in combination with hCG and estriol, as this has been shown in other studies to increase the sensitivity and the specificity of such screening.
Fjölmargar greinar hafa birst á undanförnum árum, sem sýna tengsl milli lágra AFP gilda í sermi þungaðrar konu og litningagalla hjá fóstri. Til að kanna þessi tengsl hjá íslenskum konum og til að finna viðmiðunargildi fyrir rannsóknaraðferð Rannsóknadeildar Landspítalans voru athuguð s-AFP gildi hjá 1309 konum, sem höfðu gengist undir legástungur árin 1982 til 1986. Af þeim reyndust 763 konur hafa s-AFP gildi undir viðmiðunarmörkum og fannst þrístæða 21 hjá 10 þeirra eða 1,3%; 546 konur voru með s-AFP ofar viðmiðunarmörkum og fannst þrístæða 21 hjá einni þeirra eða 0,2%. Með skimun á s-AFP gildum hjá þunguðum konum undir 35 ára aldri má bæta verulega leit að litningagöllum í meðgöngu. Rannsóknir erlendis frá benda til þess að með því að mæla einnig hCG og estríól megi auka verulega árangur þessarar skimunar. Lagt er til, að slík skimun verði tekin upp í mæðravernd á Íslandi.
