Abstract
This is a retrospective study of 189 patients with acute pancreatitis admitted to Reykjavik City Hospital during a 10-year period, 1974-1983. 35% of cases were gallstone related, 26% were alcohol related and 30% were idiopatic. Gallstone related pancreatitis increased with age and was the most frequent cause among elderly women. Men under 60 years of age comprised most of the alcohol related group. Mortality was 4.7%. The complication rate was significant, comparable to other foreign studies. There was some correlation between high amylase levels on admission and length of hospital stay. Six biochemical factors as well as age proved prognostically valuable relative to mortality and complications. However prospective study with use of clinical and biochemical factors will be necessary to obtain clear answers to their real practical value and for comparison with other similar studies.
Eitt hundrað áttatíu og níu sjúklingar á Borgarspítalanum með bráða brisbólgu voru kannaðir með eftirvirkum hætti á 10 ára tímabili, 1974-1983. Hjá þriðjungi var orsök gallvegasjúkdómar og hjá fjórðungi áfengisneysla. Um þriðjungur hafði óþekkta orsök fyrir brisbólgunni. Galltengd brisbólga verður tíðari með hærri aldri og er algengust meðal eldri kvenna, en áfengistengd bólga er algengust hjá körlum yngri en 60 ára. Dánartíðni var 4.7% og tíðni fylgikvilla há eins og hjá nágrannaþjóðum okkar. Amýlasi í sermi reyndist hærri í galltengdri en áfengistengdri bólgu. Legutími þeirra, sem höfðu háan amýlasa við komu hafði tilhneigingu til að vera lengri þótt ekki væri um tölfræðilegan mun að ræða. Forspárgildi aldurs auk sex lífefnafræðilegra þátta um horfur í byrjun sjúkdómsástands höfðu ótvírætt gildi. Orsakir og árangur meðferðar eru svipuð því sem gerist í nágrannalöndum okkar, þótt ógreindar orsakir séu mun algengari hér á landi. Framvirk rannsókn á þessum sjúkdómi, þar sem flokkunarkerfi Ransons eða Imries eru notuð, er nauðsynleg til að afla áreiðanlegra svara til samanburðar við aðrar rannsóknarniðurstöður.
